“An error occurred” When Submitting Questionnaire Form

    Whenever I fill out the form and click Submit, I always receive an “An error occurred.” message.

    I made sure to complete all fields correctly, including the required formats for questions 4 and 5. I also tried changing the question types and re-creating the form, but the same error continues to appear every time.

    Could you please look into this and let me know what might be causing the issue? I’d appreciate your help in resolving it as soon as possible.
