1) I've attached that form, so now please go to Tools > Import > WordPress, and import the attached file.
2) After you have installed and activated all the required plugins ( which you will find at Appearance > Install Plugins), install also the Import Demo Data plugin, then go to Appearance > Import Demo Data > Templates and you can import all the headers that are included in the theme, to set a header on entire site please go to Appearance > Customize > Header > Layout > Template and select the header template that you want to use.
Thank you for your quick response! I did as advised, but unfortunately I still cannot set up my website in the same way as I see in Themes' DEMO version.
1) After import of attached file I got the message: "Contact Form “Classic Form” already exists.". So I can only set up the same classic form only (which I already had), but I would like to get "Creative Style" form. In additional I used "One Click Demo Import" plugin to import "Elements" (please see attached Elements.png), as "Creative Style" form should be included (?) in this package. Unfortunately, the form is still missing (please see attached Contact forms from Elements.png).
2) I imported "Header Templates" with the "One Click Demo Import" plugin, but still I don't see the same menu settings as in your picture. Please see attached Header layout.png Also, do i understand correctly that you show how to change the top menu for the whole theme here? I want to leave the Top menu as it is in home page (shop creative), but I want to add different style menu on Contact page (I would like to get exactly the same as in demo version).
2) If you want to have the same header for the entire site, please go to Dashboard > Appearance > Customize > Header > Layout > Type, set the type to the template, and select the template that you want to use.
And if you want to have a different header for different pages, you can do that by editing the page that you want to change the header, go to the page settings > header and there you can select the header template just for that page.

But on my contact form, the headers are left visible, but I can enter text below. Not big deal, I can live with it as it is :)
2) Yes, I want to have a different header for different pages (the same as in Bifrost Demo). Thanks to your advice, I managed to add the menu on my contact page, but it is the same as on Home page. I have only two options for menu (please see attached Menu options.png) and can't choose different style of the menu. As already said, I want to add the same menu style as seen on Bifrost menu.png - can I?
